Whether it’s finding a doctor or facility, making or getting to an appointment, dealing with medical bills and insurance companies, or caring for a loved one – we want to help. There’s no cost and it’s all confidential.

La Plata Cares has the most complete, up-to-date resource directory in our area. You can easily browse the directory to find:

Our team of community health workers are trusted, local and trained to provide support, education and guidance. We have a deep understanding of our healthcare system and locally available resources.
